These Terms of Service ("Terms") govern your use of the Freedom Healthcare website and, for active members, the Freedom Healthcare membership program. By using this site or by enrolling as a member, you agree to these Terms.

If you are accepting these Terms on behalf of an organization, you represent that you have the authority to bind that organization to these Terms.

Freedom Healthcare is a physician-led, membership-based medical practice. Membership provides access to a longitudinal physician relationship and the supporting care team described elsewhere on this site.

Freedom Healthcare is not an insurance plan, is not a substitute for emergency or hospital-based care, and does not bill insurance for the services included in the membership. Members are expected to maintain appropriate coverage for care that falls outside the membership, including emergency care, hospitalization, specialist referrals, and prescription medications.

Membership is by application. Acceptance is at the sole discretion of the physician team. Application materials are reviewed within seven days; decisions (accepted, waitlisted, or returned with notes) are communicated in writing.

Prospective members pay for the initial discovery call. The fee is non-refundable. The call is reviewed in advance by the physician team and conducted as a serious clinical conversation, not a sales call. Fee details are disclosed before payment is collected.

Founding membership pricing, renewal terms, and any reservation deposit are set out in the membership agreement provided at intake. Membership fees are payable annually in advance unless otherwise agreed in writing.

We accept payment by ACH transfer or major credit card. Failed or declined payments may result in suspension of non-urgent services until the account is brought current.

Members may cancel the membership at any time by written notice. Refunds are pro-rated for unused calendar months of the prepaid term, less an administrative reserve documented in your membership agreement. There are no early-termination penalties.

The reservation deposit paid to join the founding cohort is applied to the first year of membership and is non-refundable once an acceptance offer has been signed.

This site is provided for informational purposes for prospective and active members. You agree not to interfere with the operation of the site, attempt to access non-public areas, reverse engineer any portion of the service, or use the site for purposes inconsistent with these Terms or applicable law.

All content on this site, including text, graphics, logos, and images, is the property of Freedom Healthcare or its licensors and is protected by United States and international copyright laws. You may not reproduce or republish this content without written permission.

Information published on this site is for general informational purposes only and is not medical advice. Reviewing this site does not create a physician-patient relationship. The physician-patient relationship is established only after acceptance into the membership and execution of the membership agreement.

To the maximum extent permitted by law, Freedom Healthcare is not liable for any indirect, incidental, consequential, special, or punitive damages arising from your use of this site. Nothing in these Terms limits any liability that cannot be limited under applicable law, including liability for personal injury caused by negligence or for fraudulent misrepresentation.

These Terms are governed by the laws of the State of Florida, without regard to its conflict of laws principles. Any dispute arising out of or relating to these Terms or the Freedom Healthcare website is subject to the exclusive jurisdiction of the state and federal courts located in Miami-Dade County, Florida. Disputes relating to the membership itself are governed by the dispute resolution clause of the membership agreement.

We may revise these Terms from time to time. Material changes will be communicated to active members in writing at least thirty days before they take effect. The Last Updated date at the top of this page will be revised when a new version is published. Continued use of the site after a revision constitutes acceptance of the revised Terms.
