Freedom Healthcare ("we," "us," or "our") is committed to protecting your privacy. This Privacy Policy describes how we collect, use, and safeguard your information when you visit our website, communicate with us, or become a member of our practice.

01.Information We Collect

We may collect personal information such as your name, email address, phone number, mailing address, date of birth, and emergency contact details when you visit our website, request information, apply for membership, or use our services.

When you become a member, we also collect protected health information (PHI): clinical history, diagnostic and lab results, medications, treatment plans, and the records generated through your physician relationship. PHI is collected only for the purpose of providing care.

We do not run third-party advertising pixels or behavioral trackers on this site. We do not buy personal information about you from data brokers.

02.How We Use Your Information

We use your information to provide and improve our services, communicate with you, process payments, and comply with legal and regulatory obligations.

Clinical information is used by your physician and care team to deliver, document, and coordinate your care. Application information is used by the physician team to evaluate fit. Operational information is used to run the practice. We do not use personal information for marketing to third parties.

03.How We Protect Your Information

We implement administrative, technical, and physical safeguards designed to protect personal information from unauthorized access, alteration, disclosure, or destruction.

Clinical records are stored in a HIPAA-aligned electronic medical record. Non-clinical information is stored in systems chosen for their handling of sensitive data. Access is role-based, audited, and limited to staff with a documented need to know.

04.Information Sharing

We do not sell or rent your personal information. We may share information with trusted service providers (such as our electronic medical record vendor, payment processor, and secure messaging platform) who assist in operating the practice and are contractually obligated to keep your information confidential and secure.

We may share clinical information with other clinicians at your written direction, when legally required (for example, a court order or public health reporting obligation), or when reasonably necessary to prevent imminent harm.

05.Your Rights

You have the right to access, correct, or request deletion of your personal information. You may also object to or restrict certain uses of your information, subject to applicable legal retention requirements that apply to medical records.

06.HIPAA Notice of Privacy Practices

Freedom Healthcare is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We maintain a separate Notice of Privacy Practices that describes in detail how protected health information about you may be used and disclosed, and how you can obtain access to that information.

A current copy of the Notice of Privacy Practices is provided at intake and is available at any time on request. By signing the membership agreement, you acknowledge receipt of the notice.

07.Cookies and Analytics

This site uses a small number of strictly necessary cookies required for the site to function (for example, to remember a session or a form submission). It does not use third-party advertising cookies.

We use Google Analytics 4 to understand, in aggregate, which pages are most useful to prospective members — for example, how many visitors a page receives and how they arrived at the site. The data we review is aggregated and pseudonymized; IP addresses are not stored, and nothing you type into any form on this site is ever sent to analytics. No medical or health information is collected by, or shared with, analytics tooling.

08.Children's Privacy

This site and the Freedom Healthcare membership are directed to adults. We do not knowingly collect information from children under 13. If you believe a child has provided personal information to us, please contact us so we can delete it.

09.Updates to This Policy

We may update this policy from time to time to reflect changes in our practices or applicable law. Material changes are communicated to active members in writing at least thirty days before they take effect. The Last Updated date at the top of this page will be revised when a new version is published.
